When a Loved One Passes On at Home

Home may be a place where patients choose to spend their final days of life. When a loved one passes on at home, our doctors do home visits to help families issue Certificate of Cause of Death (CCOD) for individuals who pass on due to known and natural causes.

What is a CCOD?

A Certificate of Cause of Death (CCOD) is obtained from a certified doctor.

Doctors can only issue a CCOD when the cause of death is known and natural. If the cause is of death is unnatural, the body shall be handed over to the police for further investigation.

Family members are required to register the death within 24 hours.

What is a CCOD needed for?

A CCOD is needed for the following:

– Place an obituary in the newspaper
– Engage a funeral director
– Make arrangements for cremation or burial

How does it work?

  • Step 1: Book a doctor’s visit with us here
  • Step 2: Our doctor will conduct the assessment and certify the death
  • Step 3: Our doctor will submit the CCOD digitally at your home
  • Step 4: Our doctor will guide you on how to download the digital certificate through My Legacy website

Note:

The price provided below is inclusive of 9% GST and is only applicable for services rendered from 1st January 2024 onwards.

Additional $108 (after GST) applies for removal of pacemaker or other implanted device. If the deceased is to be cremated upon their passing, pacemaker removal is required.
